PLEASE NOTE: It is the client’s responsibility to know their own out-of-pocket cost-share for services based on the benefits they receive. Our practice will do a cursory confirmation of benefits before services start, but it’s ultimately the client’s responsibility to know what their plan outlines in terms of copays, deductibles, and more generally cost-share responsibilities.

Tricare (‘Certified Out of Network’ status, ask your PCM for details prior to starting)
Anthem

If you have an insurance plan that's not on the list it's possible your therapist can still be considered an "out of network provider" and as such you could be reimbursed for some of their fee.  Call the member services number on the back of your insurance card to inquire.
